Wesley Medical Center, part of the HCA Midwest Health system, is seeking a compassionate and detail-oriented Transplant Coordinator RN to support our growing transplant services program in the Healthcare & Medical Services department. In this role, you will coordinate and deliver comprehensive nursing care to transplant candidates, recipients, and living donors across the entire continuum—from initial evaluation and pre-transplant workup, through the perioperative period, to long-term post-transplant follow-up. The Transplant Coordinator RN serves as a clinical expert and primary point of contact for patients and families, providing education, counseling, and ongoing support to help them understand the transplant process, manage complex medical regimens, and navigate the emotional and practical challenges of transplantation. You will collaborate closely with surgeons, physicians, advanced practice providers, social workers, pharmacists, dietitians, and other members of the multidisciplinary transplant team to ensure safe, timely, and coordinated care. Key aspects of this role include organizing and tracking diagnostic testing and consultations, assessing patient status and transplant readiness, participating in patient selection and listing discussions, and helping to coordinate organ offers and allocation in accordance with regulatory and ethical standards. You will assist in developing individualized plans of care, monitor for complications or signs of rejection or infection, and support early identification and escalation of changes in patient condition. The Transplant Coordinator RN is also responsible for meticulous documentation, regulatory compliance, and quality improvement activities related to transplant outcomes and patient safety. You will help maintain accurate records for internal review and external reporting, contribute to protocol development and implementation, and support education for staff, patients, and the community about transplantation and organ donation.
