Job Title: Registered Nurse - Case Management

Location: Concord, CA

Department/Unit: Case Management

Salary Range: $50 to $60 per hour

Shift: 40 hours/week, 7:00 AM to 3:30 PM

Contract Duration: 13 Weeks

Elitecare Medical Staffing is seeking a dedicated and experienced Registered Nurse - Case Manager to join our dynamic team in Concord, CA. The ideal candidate will have recent experience in acute care hospital case management and a passion for delivering exceptional patient care.

• Provide comprehensive case management services to patients in an acute care hospital setting.
• Float between Concord and Walnut Creek as needed, adhering to the local travel policy (50-mile radius).
• Collaborate with healthcare providers, patients, and their families to develop and implement effective care plans.
• Utilize the Epic Electronic Medical Record (EMR) system for documentation and communication.
• Ensure compliance with all regulatory requirements and organizational policies.
• Participate in interdisciplinary team meetings to discuss patient progress and adjust care plans as necessary.

• Licensure: Current California Registered Nurse (RN) license required at the time of submission.
• Experience: Minimum of 2 years of recent acute care hospital case management experience is mandatory. Previous travel nursing experience is preferred.
• Skills: Proficient in using electronic medical records, preferably Epic. Strong communication and interpersonal skills are essential.
• Ability to work collaboratively in a fast-paced environment while managing multiple priorities.
