Job Description
POSITION SUMMARY
A medical care manager (MCM) provides care management services for the HIV patient population. Care management services include direct patient interaction and indirect patient assistance, education, and care team documentation. MCMs increase access to care, ensure effective care coordination, and improve outcomes for the HIV/AIDS patient population.
KEY RESPONSIBILITIES
Know current Ryan White (RW) eligibility criteria
Obtain/maintain required RW documents in assigned patients’ charts
Assess the medical/social needs of patients by completing psychosocial assessments
Make necessary referrals according to psychosocial assessment results
Take initiative in creating client-centered care plans
Follow through with care plans every 6 months to document referral outcomes
Manage time efficiently enough to both meet with scheduled patients and complete necessary visit documentation/data
Accurately/precisely enters information into the electronic health record and CAREWare systems according to grant reporting needs
Works cooperatively with medical providers, early intervention specialists, external support agencies, and other MCMs
Effectively and patiently works with patients
Attends local/state planning meetings and training programs as advised by supervisor
Performs other duties as required
POSITION REQUIREMENTS
Education: minimum bachelor’s degree in social work (or related field)
Experience: minimum 1 year in social work or care management; minimum 1 year working with persons diagnosed with HIV/AIDS
Skills/abilities: proficiency in computer use, keyboarding, and Microsoft Windows/Office programs
Mental capacity:
LEVEL2=requires intermittent high concentration and normal attention; generally, once learned, job tasks still require attention to recurring variables
Physical capacity:
Sitting 80%, Standing 10%, and Walking 10% of time
Lifting, pushing, or pulling 15% of time; max 25lbs of equipment, paper, or other materials
Machines/equipment (hand/eye coordination):
Computer--80% of time (normal)
Phone--10% of time (normal)
Copier--5% of time (normal)
Other typical office equipment--5% of time (normal)
On-the-job travel:
15% of time traveling, excluding regular commute to primary location(s)
WORKING CONDITIONS
Healthcare clinic setting with frequent exporsure to contagious patients, possible exposure to infectious diseases (bloodborne and airborne), and proxy risk from biohazard waste and equipment; required universal precautions and attention to privacy; recommended attention to prevent repetitive use injuries. Home visits to urban neighborhoods may include exposure to varying cultures, socioeconomic levels, and weather conditions.
