Job Summary
As a Behavioral Health Care Manager, you will play a critical role in supporting care management and coordination by collaborating with a multidisciplinary team to ensure the delivery of integrated member care. You will strive to facilitate positive member outcomes and enhance the overall strategy for providing quality and cost-effective care.

Essential Job Duties

• Conduct comprehensive behavioral health assessments within regulated timelines to identify members who qualify for care coordination or case management.
• Collaborate with members, caregivers, physicians, and healthcare professionals to develop and implement personalized care plans addressing member needs and goals.
• Perform assessments through telephonic, face-to-face, or in-home visits as required.
• Monitor and evaluate the effectiveness of care plans, documenting interventions and progress towards goals while suggesting adjustments as necessary.
• Maintain an ongoing caseload for regular outreach and management of members.
• Promote the integration of behavioral health services, long-term services and supports (LTSS), and community resources to ensure continuity of care.
• Facilitate interdisciplinary care team meetings and foster informal collaboration among the team.
• Utilize motivational interviewing techniques and Molina clinical guideposts to support and motivate members during interactions.
• Identify and assess barriers to care, providing assistance and coordination to address member concerns.
• Offer consultations, resources, and recommendations to peers as needed.
• Be prepared for 25-40% local travel based on state and contractual requirements.

Required Qualifications

• Minimum of 2 years of healthcare experience, ideally in behavioral health, or a related combination of education and experience.
• Active and unrestricted license in behavioral health, including but not limited to: LCSW, LMSW, APSW, CHES, LPC, LPCC, LMFT, PhD, or PsyD based on state regulations.
• Reliable transportation, valid driver's license, and appropriate auto insurance for job-related travel, as required by law.
• Experience working with individuals facing severe mental health challenges, serious emotional disturbances, and substance use disorders.
• Understanding of whole person care principles, chronic health conditions, and discharge planning coordination.
• Proficient data entry skills and prior experience with clinical platforms.
• Exceptional verbal and written communication abilities.
• Proficiency in Microsoft Office and relevant software applications.

Preferred Qualifications

• Certification as a Case Manager (CCM).
• Experience in managing behavioral health care.
• Field-based care management or home health experience.

Pay Range: $25.08 - $51.49 / HOURLY
